A defasciculating dose of d-tubocurarine causes resistance to succinylcholine.

چکیده
Forty-four patients, ASA physical status I or II, undergoing thiamylal, fentanyl, N2O/O2 anaesthesia were studied to determine the dose-response to succinylcholine (Sch) without prior defasciculation (24 pt - Group 1), or three minutes following d-tubocurarine (dTC), 0.043 mg.kg-1 (20 pt - Group 2). The individual log dose-logit response curve for each patient was determined using a cumulative dose plus infusion technique and integrated EMG monitoring of the first dorsal interosseous muscle. The mean (+/- SEM) ED50, ED90 and ED95 values for Sch in Group 1 were 0.13 +/- 0.01, 0.19 +/- 0.01 and 0.22 +- 0.01 mg.kg-1, and in Group 2 were 0.16 +/- 0.01, 0.25 +/- 0.01 and 0.29 +/- 0.02 mg.kg-1, respectively. The mean ED values in Group 2 were significantly greater than the equivalent values in Group 1 (P less than 0.05). Compared with values in Group 1, ED values in Group 2 represented mean increases of 23, 32, and 32 per cent, respectively. These pharmacodynamic data indicate that the dose of Sch needs to be increased by 32 per cent following a defasciculating dose of dTC 3 mg.70 kg-1 (0.043 mg.kg-1).

The Effect of Defasciculating Doses of Pancuronium and Atracurium on Succinylcholine Neuromuscular Blockade
BACKGROUND A defasciculating dose of non-depolarizing muscle relaxant administered prior succinylcholine decrease its side effects including fasciculations and postoperative myalgias; however it is believed that the dosage of succinylcholine should be increased when such a pre-treatment is used. OBJECTIVES We hypothesized that a defasciculating dose of pancuronium as a pre-treatment could pro...
متن کاملEffect of atropine on bradydysrhythmias induced by succinylcholine following pretreatment with D-tubocurarine.
This study was designed to compare the effectiveness of pretreatment with the combination of d-tubocurarine and atropine with d-tubocurarine alone in preventing changes in cardiac rate and rhythm following repeated administration of succinylcholine. Sixty subjects were randomly divided into three groups of twenty. Group one received d-tubocurarine 0.04 mg.kg-1 and atropine 0.01 mg.kg-1, and gro...
متن کاملIn patients with head injuries who undergo rapid sequence intubation using succinylcholine, does pretreatment with a competitive neuromuscular blocking agent improve outcome? A literature review.
A literature search was undertaken for evidence of the effect of succinylcholine (SCH) on the intracranial pressure (ICP) of patients with acute brain injury and whether pretreatment with a defasciculating dose of competitive neuromuscular blocker is beneficial in this patient group. The authors could find no definitive evidence that SCH caused a rise in ICP in patients with brain injury. Howev...
متن کاملThe role of the non-depolarizing drugs in the prevention of suxamethonium bradycardia.
Evidence has been presented to show that a second dose of suxamethonium is capable of producing a severe bradycardia in man. This response may be effectively prevented by the prior administration of certain non-depolarizing muscle relaxants namely tubocurarine, alcuronium, c-toxiferine and pancuronium in quantities of one-quarter or less of their muscle relaxant level. It is suggested that suxa...
متن کاملEffects of prostaglandins and indomethacin on neuromuscular blocking agents.
The effects on neuromuscular blockade by d-tubocurarine and succinylcholine of inhibition of prostaglandin biosynthesis by indomethacin and of intra-arterial administration of prostoglandins E2 and F2 alpha, before and after inhibition of prostoglandin biosynthesis, were evaluated in the cat sciatic-tibialis preparation. Non-specific inhibition of prostaglandin biosynthesis by indomethacin 3 mg...
